Child Health Outreach

Care Management for At-Risk Children (CMARC)

This service is provided at no cost for children ages birth to five years old who have Medicaid or are uninsured and are at risk for health or developmental problems. The CMARC care manager is a public health nurse with the community health program who will:

  • assess the needs of your child and family
  • provide referrals to services
  • give information to the family about development and parenting.

Newborn Home Visit

After delivery, a public health nurse is available to visit the new mother and her baby in the home to:

  • assess the mother and baby
  • offer feeding advice and breastfeeding support
  • determine other needs
  • assist with referrals for other services.
